Aggregate Stop Loss Insurance in the Amount of two hundred eighty-five thousand four hundred twenty-six dollars ($285,426) to Aetna in the estimated amount of eleven thousand
dive hundred ninety-two dollars ($11,592) for Section 125/Cafeteria Plan Administration to Delta Dental for dental insurance in the estimated amount of one hundred ninety-nine thousand two hundred ninety-six dollars ($199,296) to CIGNA for life accidental death insurance optional life and accidental death insurance longterm disability and optional shortterm disability in the estimated amount of ninety-six thousand three hundred nineteen dollars ($96,319) to Alliance
Workplace Partners in the estimated amount of six thousand five hundred eighty-nine dollars ($6,589) for an Employee Assistance Program Optional Vision Insurance to Block Vision in the
amount of eighteen thousand seven hundred thirty-five dollars ($18,735) and authorization for the City Manager to execute all necessary documents.
2B. Consider an ordinance establishing a minimum reserve for the new Employee Health Benefits Fund and transferring five hundred eleven thousand three hundred thirty-one dollars ($511,331)
from the General Fund for this purpose effective October 1, 2010.
